For your ROSTER to be included it :

- must be submitted to our office no later then Nov 14th, 2014

- must be in the format we have provided

- document must be saved with your Team Name and Division i.e. Avon 4Boys

A Waiver form must be turned in for each participant. No Exceptions!!

Please fill out and hand these form in before each player can play.
